Reconstitution

Here are the reconstitution procedures for tirzepatide:

Reconstitution of 5mg Vial

In order to obtain standard dosing, the reconstitution ratio is one milliliter (1ml) of bacteriostatic water in 5mg of tirzepatide. This is equal to 100 units on a standard syringe.

Supplies needed:

  • One (1) 5mg vial of tirzepatide
  • One (1) bottle of bacteriostatic water
  • One (1) syringe of at least 1ml in volume
  • Several alcohol prep pads for disinfection use

Procedure:

  1. Place all components on a sterile surface.
  2. Remove cap from the 5mg tirzepatide vial.
  3. Remove cap from the bottle of bacteriostatic water.
  4. Using an alcohol prep pad, sterilize the now exposed top of the 5mg tirzepatide vial.
  5. Using an alcohol prep pad, sterilize the now exposed top of the bacteriostatic water bottle.
  6. Using a fresh syringe, extract 1ml of bacteriostatic water from the bottle.
  7. Add the bacteriostatic water into the 5mg tirzepatide vial.
  8. Once 1ml has been added to the 5mg tirzepatide vial, remove the syringe and dispose of it safely.
  9. Gently roll the vial to dissolve the tirzepatide into the bacteriostatic water. DO NOT SHAKE. Gently roll until dissolved entirely.
  10. Store the reconstituted solution in a refrigerated environment (DO NOT FREEZE.)

Once reconstituted, the solution has a shelf life of no longer than 8 weeks (56 days.)

After 8 weeks, dispose of the vial safely and appropriately.

Dose chart from this vial:

  • Week 1, 2, 3, 4 – 2.5mg of tirzepatide (50 units on a standard syringe)
  • Week 5, 6, 7, 8 – 5.0mg of tirzepatide (100 units on a standard syringe)

Reconstitution of 10mg Vial

In order to obtain standard dosing, the reconstitution ratio is one milliliter (1ml) of bacteriostatic water in 10mg of tirzepatide. This is equal to 100 units on a standard syringe.

Supplies needed:

  • One (1) 10mg vial of tirzepatide
  • One (1) bottle of bacteriostatic water
  • One (1) syringe of at least 1ml in volume
  • Several alcohol prep pads for disinfection use

Procedure:

  1. Place all components on a sterile surface.
  2. Remove cap from the 10mg tirzepatide vial.
  3. Remove cap from the bottle of bacteriostatic water.
  4. Using an alcohol prep pad, sterilize the now exposed top of the 10mg tirzepatide vial.
  5. Using an alcohol prep pad, sterilize the now exposed top of the bacteriostatic water bottle.
  6. Using a fresh syringe, extract 1ml of bacteriostatic water from the bottle.
  7. Add the bacteriostatic water into the 10mg tirzepatide vial.
  8. Once 1ml has been added to the 10mg tirzepatide vial, remove the syringe and dispose of it safely.

  • Week 1, 2, 3, 4 – 2.5mg of tirzepatide (25 units on a standard syringe)
  • Week 5, 6, 7, 8 – 5.0mg of tirzepatide (50 units on a standard syringe)
  • Week 9, 10, 11, 12 – 7.5mg of tirzepatide (75 units on a standard syringe)
  • Week 13, 14, 15, 16 – 10.0mg of tirzepatide (100 units on a standard syringe)

Reconstitution of 15mg Vial

In order to obtain standard dosing, the reconstitution ratio is one point two milliliters (1.2ml) of bacteriostatic water in 15mg of tirzepatide. This is equal to 120 units on a standard syringe (or 100 units plus 20 units.)

Supplies needed:

  • One (1) 15mg vial of tirzepatide
  • One (1) bottle of bacteriostatic water
  • One (1) syringe of at least 1ml in volume
  • Several alcohol prep pads for disinfection use

Procedure:

  1. Place all components on a sterile surface.
  2. Remove cap from the 15mg tirzepatide vial.
  3. Remove cap from the bottle of bacteriostatic water.
  4. Using an alcohol prep pad, sterilize the now exposed top of the 15mg tirzepatide vial.
  5. Using an alcohol prep pad, sterilize the now exposed top of the bacteriostatic water bottle.
  6. Using a fresh syringe, extract 1ml of bacteriostatic water from the bottle.
  7. Add the bacteriostatic water into the 15mg tirzepatide vial.
    • If using a 1ml syringe, repeat steps 5, 6, and 7 with 0.2ml of water (20 units) until the 15mg tirzepatide vial contains 1.2ml (120 units) of bacteriostatic water.
    • In order to prevent cross contamination, do not use the same syringe twice.
  8. Once 1.2ml (120 units) has been added to the 15mg tirzepatide vial, remove the syringe and dispose of it safely.

  • Week 1, 2, 3, 4 – 2.5mg of tirzepatide (20 units on a standard syringe)
  • Week 5, 6, 7, 8 – 5.0mg of tirzepatide (40 units on a standard syringe)
  • Week 9, 10, 11, 12 – 7.5mg of tirzepatide (60 units on a standard syringe)
  • Week 13, 14, 15, 16 – 10.0mg of tirzepatide (80 units on a standard syringe)
  • Week 17, 18, 19, 20 – 12.5mg of tirzepatide (100 units on a standard syringe)
  • Week 21+ – 15.0mg of tirzepatide (120 units on a standard syringe – or 100 units and supplement with another 20 units.)
